System now facilitates you create customized forms for patients. Practice may want to send some generalized forms to all the patients in that case the patient forms will be very useful. You just need to send these forms to the patient portal users. The patients can fill these forms and send them back to the practice.

 

Note: To enable this feature for your practice the patient forms should be first configured.

To search patient forms that were sent/received, do the following:

1.From left menu items, select Patients > Patient Forms.
2.In the search panel, enter one of these fields to perform the patient form search: Patient Name, Form, Category, Sent From/To date, Status.
3.Select the "Show only filled Forms" check box to search only forms that were filled by the patients.
4.Click Search to perform the patient form search.
5.Click Rename Form icon Rename Form icon, to rename the form received from the patients.

 

You can also configure patient forms as Dynamic Forms. Once you configure dynamic forms for patient forms, they will appear in the Patient Forms section. On the Patient Forms screen, you can see a list of configured forms. The list will have the custom clinical forms and the dynamic forms. Custom clinical forms will appear in red with (old CCFs) tag after the form name. This will help you to quickly differentiate old and new forms.
