Adding an Amendment Request

Patients may have some queries concerning their recent visit to the practice; e.g., a patient may want to continue taking insulin for one more month or want to change the medication due to side effects. InSync has a provision to entertain such patient queries through the Amendment Request feature.

 

Note: You must configure the “Patient Amendment Request on Encounter Note” option in the Practice Defaults section. When configured, the “Amendment Request / Addendum” panel appears; otherwise, only the “Addendum” panel is displayed.

 

The user can capture the amendment request details, such as who has requested the amendment, when the request was sent, amendment request description, and its status. Initially the request can be entered with Pending status, which can be later accepted or denied by entering comments. Upon saving the amendment request, the details are stored in the subsequent grid. Once the request is accepted or denied, it is displayed on the Encounter note.

 

To add an amendment request:

1.On the Facesheet screen, click the Addendum_Attached icon corresponding to the encounter for which you want to add an amendment request.
2.Click the closed encounter iconend encounter from the icons displayed in top right corner. A screen appears with Encounter Note, Co-Signature, Amendment Request / Addendum, and Patient Signature panels.
3.Expand the Amendment Request / Addendum panel.
4.Enter the amendment notes and click Save. The amendment details are displayed at the end on the Encounter note as shown in the following screen.
5.In the Amendment Request Source field, the patient’s name appears by default. This is a free-form text field. You can change the name, in the case the request is received from someone else (e.g., the patient’s representative.)
6.By default, the current date is displayed in the Amendment Request Date field. If required, you can change the date using the calendar button. It is free-form text field. If the exact date is not remembered, you can type any text in this field.
7.In the Amendment Request Description field, describe the request details.
8.Enter accept or denial Comments.
9.Click Save.
10.The entered details will be displayed in the subsequent grid.
11.When the amendment request is accepted or denied, the Encounter note is updated accordingly.
